Bariatric surgery is a surgical breakthrough to promote effective weight loss which can be divided into two categories; restrictive or malabsorptive. Restrictive procedures work by physically restricting the size and the capacity of the stomach as well as decreasing the rate of digestion. Malabsorptive procedures, on the other hand, modify the absorptive capacity of the digestive system by physically removing parts of the digestive tract which greatly limits calorie absorption. Figuring out which is the best surgical alternative for you is more than a decision but a life

_What is Obesity? Obesity results from the excessive accumulation of fat that exceeds the body's skeletal and physical standards. According to the National Institutes of Health (NIH), an increase in 20 percent or more above your ideal body weight is the point at which excess weight becomes a health risk. Today 97 million Americans, more than one-third of the adult population, are overweight or obese. An estimated 5 to 10 million of those are considered morbidly obese. For people who are fed up of seeing their weight off the scales, there’s another way to lose the extra pounds, not just temporarily but permanently.

Now, there is HOPE for OBESITY! Bariatric surgery

After
Bariatric Surgery is derived from the greek word “baros” meaning pressure and “iatros” which refers to science and knowledge. When you are obese you have so much pressure within your body, physically, mentally & emotionally.

According to Dr. Hildegrades C. Dineros, a pioneer and leader in the field of Metabolic and Bariatirc Surgery in the Philippines and a Fellow of the American College of Surgeons (ACS), and Philippines College of Surgeons (PCS), currently the President of the Philippines Society for Metabolic and Bariatric Surgery, “obesity is when you take in more than what you should eat.  It is not when, why and how you take in food, it is still at the end of the day, the between intake and expenditure of calories”.

We must be motivated by nutrition, for beyond that, it is over eating for pleasure.  It is okay once is a while to over eat but when it becomes habitual, that you tend to take in more than you should, then you have the tendency to become obese”.  He added that habitual over eating is a sign of food addiction.

The treatment of obesity is tailored according to the patient’s BMI category, health conditions, and patient’s preferences. His Obesity Center offers a wide range of treatment options including non-surgical management obesity.

The foundation of treatment is compliance to lifestyle modifications including a nutritional intervention and progressive physical activity.

Unfortunately, diet and exercise may not  be adequate to attain the desired weight loss especially in the morbidly obese patients. Thus, bariatric surgery is the best and the most durable treatment option for these set of patients including  with diabetes.

Bariatric Surgery is an operation designed to help you lose weight.  It is  anchored on two basic principles. One is RESTRICTIVE; limiting the amount of food that is ingested, and the other is MALABSORPTIVE, decreasing the chance for ingested food to get into the body.  Some procedures utilize both mechanisms.  Depending on the compliance of a patient 50-100% of the excess weight maybe lost after the surgery.  A patient usually looses one thrird of his/her obese weight in a year.

_The most popular procedure is the Gastric Bypass which limits food in take and has some malabsorption.  Gastric Banding is increasingly performed worldwide, while Sleeve Resection of the stomach is becoming known. Both are purely restrictive procedures.

Biliopancreatic Diversion with Doudenal Switch is more malabsorptive but also restrictive. These procedures are usually done using LAPAROSCOPIC (minimal acess) techniques without the need for long abdominal incisions.  This permits the patient to go home after only a brief stay in the hospital.

In science, they could only see the possible causes, it can only determine how it happens and not why it happens. In science, it is more on evidenced based, physical and tangible, but for Dr. Dineros, if there’s nothing on it, it doesn’t mean it doesn’t exist.

Obesity-related health conditions are health conditions that, whether alone or in combination, can significantly reduce your life expectancy and there are more than 30 diseases related on it.  These may involve practically any organ and system of the body, from head to toe.

A severely obese person would probably   have heart disease,  hypertension, Type 2    diabetes, osteoarthritis especially of weight-bearing joints, sleep apnea, respiratory problems, gastroesophageal reflux,  heartburn,  urinary stress incontinence, gallstones, high cholesterol and triglycerides  and some form of depression  and  diminished person’s self-esteem.

The   obese   individual   would have     even a higher risk for cancer, strokes, kidney damage, liver problems and accidents. Just being obese may cause infertility including menstrual irregularities for women.

Cost Effectiveness!
With all the obesity-related health conditions mentioned, there is really a cost effectiveness in undergoing bariatric and metabolic surgery.  Imagine with all those burden obesity-related health conditions, the expenses on medications and treatments of obesity-related health conditions would even cost you more than  ten times compared to having a bariatric surgery and metabolic surgery.
